Welcome. Sarah Woolson's counseling practice is transitioning to The Listening Collective beginning November 1st, 2026.
This page is here to make the transition as simple as possible and to give you one place to find the information and steps you need.
The most important thing to know: your therapist and your ongoing counseling relationship are staying the same. The primary changes are to the business and administrative side of your care.
Please review the information below and complete the transition steps before your first appointment under Woolson Wellness.
What’s changing?

Your counseling services will now be provided through The Listening Collective, the business through which Sarah provides counseling, music therapy, therapeutic sound, and other professional services.
Your counseling relationship will transition from Transformation Counseling to The Listening Collective.

Please update your contacts accordingly.

Your counseling fee and cancellation/no-show policy will change as part of the transition.
Your individual fee and effective date are provided in your transition/fee-change letter. Please refer to that letter for the information specific to you.
You'll also review an updated Financial Policy as part of your transition paperwork.

You'll complete updated counseling paperwork through TherapyNotes.

Notice of Privacy Practices
Payment information/authorization
Other applicable practice forms
You do not need to recreate your therapeutic history from scratch. Your existing clinical information remains part of your ongoing care.

Payment for counseling will transition to The Listening Collective TherapyNotes account. You may need to re-enter your payment information.
TherapyNotes will continue to be your client portal for secure communication, scheduling, and other practice-related functions.
Specific instructions will be provided as the transition takes place.

Your individual fee and effective date are included in your transition letter.
03 Complete your client paperwork
Required forms will be available through TherapyNotes.
04 Update your contact and payment information
Make sure your contact information in TherapyNotes is up to date. You may need to re-enter your payment information in The Listening Collective TherapyNotes account.
05 Review privacy and communication information
Please review Notice of Privacy Practices, Communication Policy, and Emergency & Crisis Policy.
Communication during the transition
As part of the transition, The Listening Collective is establishing clear communication pathways so that your information is handled through the most appropriate channel.
For clinical information: Please use secure TherapyNotes messaging.
For administrative matters: You may contact Sarah by email or text for brief matters such as scheduling or rescheduling.
Please do not send clinical information by email, text, or voicemail.
The Listening Collective is not an emergency or after-hours service. If you are experiencing an emergency, call 911, go to an emergency department, or call/text 988.

The Listening Collective is the business through which Sarah will provide her counseling practice as well as her music therapy, therapeutic sound, and other professional services.
Bringing these areas of work together under one practice allows Sarah to continue developing the different parts of her professional work while maintaining a clear distinction between clinical counseling and wellness services.

Yes. Your counseling rate will change as part of the transition.
Your individual rate and effective date are included in your transition/fee-change letter. If you have questions about your individual rate, please contact Sarah.

Yes, for brief administrative matters such as scheduling or rescheduling.
Please use secure TherapyNotes messaging for clinical communication. Do not send detailed clinical information or sensitive mental health information by text, email, or voicemail.
Sarah generally responds to messages during regular business hours within 1–2 business days.

The Listening Collective provides outpatient counseling and is not an emergency, crisis, or after-hours service.

Yes. The Listening Collective is a self-pay counseling practice and can provide superbills for clients who wish to submit them to their insurance company for potential reimbursement.
The Listening Collective does not submit insurance claims on your behalf.

Your existing clinical information remains part of your ongoing care, subject to applicable privacy and recordkeeping requirements.
You do not need to recreate your entire clinical history simply because the practice is transitioning.
You’re still in the right place.
The goal of this transition is to change the business and administrative structure of your care while keeping your counseling experience as continuous and straightforward as possible.
Your counseling relationship with Sarah continues through The Listening Collective.
Once your client paperwork and payment information are complete, you can continue your counseling appointments as usual.
